Questionnaire

Please fill-in this form prior to bringing your dog for their Meet & Greet appointment. If you have not yet scheduled an appointment, a member of Mr Woofles’ team will be in touch soon.

The appointment will last for 1 hour in total, the owner is required to leave for the duration of the assessment so we can assess if your dog is suited to our open-play daycare environment. Please send a copy of your dog’s vaccination records prior to the appointment.

Requirements:

– Dogs must be fully vaccinated for DHPP and we strongly encourage that dogs attending daycare are vaccinated for Kennel Cough

– dogs must be over 16 weeks old and must have completed their full set of vaccinations

– dogs 24 months old and over must be spayed or neutered

– we are a cage free, open play daycare and dogs joining our daycare/drop-in programme must be social with no history of aggressive behaviour

The appointment fee is $15
